PHP Shortlisted: HSJ Partnership Awards 2026

Personal Homecare Pharmacy has been recognised as a finalist in two categories at this year’s HSJ Partnership Awards, a mark of the sustained, evidence‑based work we deliver with our NHS partners to improve clinical homecare and patient outcomes. The awards celebrate collaboration with the NHS that delivers measurable benefit for patients, clinicians and the wider system.

Awards Categories

Patient Safety Collaboration of the Year
This category highlights work that strengthens systems to reduce harm and improve safety in partnership with NHS teams, a central focus as we continue to refine care delivery practices.

Most Effective Contribution to Improving Care for Those with Long‑term Conditions
Recognising efforts that support consistent, integrated care for people living with long‑term conditions, this shortlist reflects our ongoing work with Leeds Teaching Hospitals NHS Trust to refine homecare pathways and support patients managing complex therapies.

About the HSJ Partnership Awards

The HSJ Partnership Awards recognise outstanding collaboration between NHS organisations and their partners, celebrating initiatives that demonstrate measurable improvements in patient care, safety and service delivery. The awards highlight projects that deliver real-world impact through partnership working, innovation and a shared commitment to improving healthcare outcomes.

Train and support our teams to deliver a consistent experience

Use data and feedback to identify where improvements are needed

Keep patients informed about their medicines and deliveries

Support hospitals and clinics with clear points of contact and reliable service

Across our operations, our focus remains on getting the basics right: safe dispensing, accurate prescriptions, reliable home delivery and communication that patients and clinicians can trust. The CSE Standard confirms that these foundations are in place and working in practice.

A Continued Focus on Patient Impact

Our inclusion in both categories underscores reliable delivery, clinical alignment and meaningful collaboration, not awards rhetoric. These recognitions are rooted in measurable service improvements and safety outcomes that matter in everyday clinical practice.

We look forward to presenting to the judging panel this January and sharing insights from the event with NHS partners and stakeholders once the process concludes.

Looking Ahead

In the weeks ahead, we will be preparing for the next stage of the awards process, including presenting to the judging panels, and continuing to strengthen the practices highlighted in this submission. At the same time, we will focus on applying the learning from this partnership more broadly across our services.

This recognition reinforces the importance of day-to-day collaboration, not just one-off projects, and invites us to build on what’s working well by sharing insights, refining processes, and continuing to support one another in delivering safe, reliable care.
